Your Menstrual Cycle: The Window Into Your Fertility

Your menstrual cycle is one of the most reliable indicators of your reproductive health. Pay attention to these changes.

Irregular or Absent Periods

This is one of the most significant warning signs. A normal menstrual cycle typically ranges from 21 to 35 days. If your cycles are consistently shorter than 21 days or longer than 35 days, or if they're unpredictable and vary widely from month to month, it could mean you're not ovulating regularly. Pregnancy cannot occur if eggs are not released regularly.

Sudden Changes in Your Cycle

Keep an eye out for cycles that, um, suddenly get shorter. Like if a menstrual cycle moves down to less than 21 days, that can point to a shrinking ovarian reserve. On the other hand, if your cycle length starts to stretch out, meaning longer pauses between periods, that’s also something to watch, because it may be meaningful too.

Changes in Flow

A sudden, dramatic decrease in menstrual flow, what some people call a “cliff-drop” in bleeding, can point to hormonal shifts that might be messing with your ability to conceive. On the other hand, if the flow is wildly heavy, or your periods are unusually painful , like worse than you ever had before, that can hint at problems such as endometriosis or fibroids, which in turn can disrupt fertility.

Bleeding Between Periods

Bleeding that just shows up between periods, or right after intercourse , can be one more clue that something is off, and it could be smart to talk to a doctor about it.

Signs That Your Hormones Might Be Out of Balance

Hormonal imbalances can really mess with fertility, and they show up in ways you might not immediately connect to your reproductive health. You know, like everything feels sort of related but you still don’t see the exact link.

Skin Changes

Unusual Hair Growth or Loss

When you notice a lot of dark, rough hair spreading on the face, chest, or even the back, it can be a clue for hirsutism which is basically that condition of excess hair. It often points to a hormonal imbalance, sort of. On the other hand, when hair starts thinning out, that too can be linked to hormone problems, even if it seems minor at first.

Unexplained Weight Changes

Sudden gain or sudden loss in weight, even if you do not really change your diet or exercise routine, can be connected to hormonal issues like PCOS or thyroid disorders. Those hormonal things can mess with fertility, and it is kind of a quiet signal at times.

Changes in Sex Drive

A noticeable drop in libido can be kind of connected to hormonal imbalances, that mess with fertility, and yeah it can show up in a pretty subtle way too sometimes.

Pain That Shouldn't Be Ignored

Painful Periods

While some cramping is normal, really severe pelvic pain around menstruation that messes with your regular day is not. That kind of discomfort can be a sign of endometriosis, which is when tissue kinda like the uterine lining starts growing outside of the uterus. Endometriosis can affect fertility because it can trigger inflammation , lead to scarring ,and cause anatomical reshaping that gets in the way of fertilisation as well as implantation, or in other words, where the embryo settles.

What the Numbers Say About Age

Age is the single most important factor affecting female fertility. Women are born with all the eggs they will ever have, and both the quantity and quality of these eggs decline over time.

By Age 30

Women have already lost nearly 90 percent of their primordial follicle pool.

After Age 35

Fertility begins to decline more rapidly. The possibility of conceiving becomes lower due to both the number and quality of eggs.

By Age 40

The chance of natural conception per cycle falls to about 5 percent. Only about three percent of the original egg reserve remains.

This doesn't mean pregnancy is impossible after 35. It means time becomes a more critical factor, and seeking help sooner rather than later matters more.

Lifestyle Factors That Can Affect Fertility

A few signs that fertility might be lower are tied to lifestyle things that can be nudged and changed.

Smoking and Alcohol

Smoking, too much alcohol, and even high caffeine intake can mess with fertility. I’ve had patients who were kinda shocked to hear how much their day to day habits were really affecting their chances, honestly. It’s like they didn’t connect those dots at first, and then suddenly it was obvious.

Chronic Stress

Long-term stress and anxiety can kind of disturb ovarian function, not just “feel bad”. In 2023 a study said that more than 60% of Indian women had low fertility awareness, so it’s sort of telling that this whole subject is rarely spoken about, even though it matters.

Weight Extremes

Both obesity and being underweight can disrupt ovulation.

What Often Gets Missed

Here's something that surprises many women. Many women with low fertility have no obvious symptoms at all. They have regular periods. They feel fine. They have no pain, no unusual bleeding, no skin changes. Everything seems normal. And then they start trying to conceive, and nothing happens.

This is why fertility isn't something you can always assess just by how you feel. If you've been trying for the recommended timeframe without success, that itself is reason enough to get evaluated, regardless of whether you have any other symptoms.

When to See a Doctor

If you're experiencing any of the signs I've described—irregular periods, severe menstrual pain, unexplained weight changes, hormonal symptoms—don't wait. And if you've been trying to conceive for 12 months (or six months if you're over 35), it's time to seek help.

Early evaluation can make a significant difference. A fertility assessment usually turns into blood work to see hormone levels like FSH, LH, AMH, thyroid markers, and prolactin , plus an ultrasound that checks the ovaries and uterus. Sometimes they also suggest some extra checks, like hysterosalpingography (HSG)  to verify whether the fallopian tubes are blocked or not, and to figure out what is going on.
